Emmons School District 2008 Satisfaction Survey

Purpose:

The purpose of this survey is to gain valuable feedback and perceptions about the Emmons experience from our parents and students, so as we can continuously improve our school.

Important Instructions:

• The survey is sectioned into 7 different areas of school effectiveness. Your responses are strictly confidential as no names are involved.

• Please rate each item based upon the extent at which you agree, disagree, or are neutral.

• We would like a completed survey for each student/grade to most accurately help us continuously improve upon our school.

• Enclosed is a confidential envelope that you can send back to school with your child or you can drop off in the box outside the administration office.

• We value your opinion and we will provide a summary of results to you at our open house to start the 2008-2009 school year in August.

• Your opinion counts as we will be using this process to help in our School District planning efforts for next year and beyond, with our new superintendent, Dr. Robert Machak.

• Due Date is by June 5^th, 2008.
